Membership Packet - 2006 Season

Please do not be dismayed at the number of pages. Hopefully we can get most of the paperwork done before the season starts. Please return all of the information together, not piece at a time!

The following documents are needed:

  1. Jesuit Waiver (pdf) - Mandatory to participate-can be filled out on line and print it out-much easier for us to read!!
  2. CIPP Confirmation - National registration mandatory for all players. Register and pay on line, then print a copy of receipt/registration form and the USARugby Liability Waiver. Turn in with your packet.
  3. Copy of health insurance cards - front and back on one side of an 8 ½" x 11" sheet of white paper
  4. Readable copy of a government issued identification - drivers' license, passport or birth certificate (on a separate sheet of white paper). This establishes your date of birth.
  5. Readable copy of your Jesuit student identification - this confirms your visual identity and that you are a bonofide high school student.
  6. Store (Rugby Gear) Order Form (pdf) - Complete your rugby gear order form on line and print out, even if you are only paying dues.

Coaches gear recommendations:
First year players: You need a jersey, socks and shorts (boots, as well, but you are on your own for those-football or soccer shoes will work). Find out if you like rugby before investing any further.

Returning players: Try to get a new pair of socks each year and take a good look at your shorts; if they are faded and/or shabby please replace them. We want to always look sharp out on the pitch! Additional gear and supplies are as you would like.

Warm ups are custom designed and come from overseas. We will order one time, January 31, 2006. We must order 20-25 suits to get this price.
